• 5 Days per week
• Full time 40 hours

What You’ll Do

• Provide skilled nursing care and patient education in the home setting.
• Develop and implement individualized care plans.
• Collaborate with the interdisciplinary team to ensure top-quality outcomes.

What You’ll Bring

• RN license in the state of employment (required)
• BLS certification (AHA)
• 1+ year of home health, acute, or SNF experience preferred

Pay Range: $34.38–$56.31/hour
